You do not need to make an appointment to get vaccinated at pop-up clinics listed below unless otherwise indicated. Clinics offer the COVID-19 vaccine for adults (12 years and older) and the pediatric vaccine (6 months-11 years old). Pop-up clinics also offer flu and MPX vaccines.

Patients ages 12-17 must bring both completed pages to the vaccination site if their parent/legal representative cannot be present at the time of vaccination. Children ages 6 months to 11 years must be accompanied by a parent or adult. If an adult other than the parent/legal representative is present at the time of vaccination, the parent can complete both pages to provide consent for vaccination or verbal consent can be obtained by phone from parent/legal guardian.
